Miliary Tuberculosis in the Elderly: Atypical Presentation and Fatal Outcome

Introduction: Tuberculosis is one of the oldest and most prevalent infectious diseases known to humankind; however, its disseminated form, known as miliary tuberculosis, is uncommon.
Objective: To describe the atypical clinical manifestations and characteristic radiological findings of miliary tuberculosis in an elderly adult.
Case Report: A 73-year-old male patient presented with a two-month history of cough with greenish sputum, malaise, weight loss, and shortness of breath. Complementary examinations were performed, and a chest CT scan revealed small nodular lesions suggestive of miliary tuberculosis.
Conclusions: Miliary tuberculosis is a serious condition, difficult to diagnose, requiring bronchoscopic sampling and potentially manifesting with rib, lymphadenopathy, and pancreatic involvement. Clinical suspicion necessitates early treatment while awaiting microbiological confirmation. This case illustrates the nonspecific presentation and poor prognosis of miliary tuberculosis in the elderly. It emphasizes the need for a high index of suspicion and early empirical treatment upon the appearance of characteristic radiological findings, even before microbiological confirmation.
