Revisión de criterios STOPP/START, Beers y FORTA para optimizar la farmacoterapia en el adulto mayor polimedicado.

Introducción: La polifarmacia en el adulto mayor es un problema de salud pública en aumento. Este fenómeno se asocia a eventos adversos por medicamentos, fragilidad, caídas y reingresos hospitalarios. Para responder a este reto, los científicos crearon herramientas explícitas. Estas herramientas identifican prescripciones potencialmente inapropiadas y también omisiones terapéuticas.
Objetivo: Analizar la evidencia actual sobre las herramientas STOPP/START, los criterios Beers y la lista FORTA, así como comparar sus ventajas, limitaciones y posible aplicación en Cuba.
Métodos: Se hizo una revisión narrativa de la literatura publicada entre enero de 2020 y febrero de 2026. Las bases de datos consultadas fueron PubMed, SciELO, Scopus y Google Scholar. La búsqueda combinó los términos “STOPP/START”, “Beers criteria”, “FORTA list”, “polypharmacy”, “older adults” y sus equivalentes en español. Al final se seleccionaron 45 artículos originales, guías clínicas y documentos de consenso.
Desarrollo: Los criterios STOPP/START (versión 3) detectan tanto las prescripciones inapropiadas como las omisiones, con una alta sensibilidad clínica. Los criterios Beers (actualización 2023) son muy completos y muy citados, pero no abordan el infratratamiento. La lista FORTA (versión 2024) clasifica los fármacos en cuatro categorías (A, B, C, D) de acuerdo con su balance beneficio-riesgo, lo que ayuda a priorizar.
Conclusiones: Las tres herramientas se complementan entre sí. Un uso combinado de ellas optimiza la farmacoterapia en el adulto mayor. En Cuba, la implementación sistemática resulta factible y necesaria, aunque siempre hace falta una adaptación a las limitaciones de recursos y al contexto de la atención primaria y secundaria.
